Recently, the clonal proliferation of CD34-positive fibroblasts in elastofibroma has been found in elastofibroma. In this article, we studied the pathogenesis of fibrosis in elastofibroma. Three tumors obtained from two patients were selected. One patient was a 57-year-old Japanese woman had a bilateral tumor and the remaining patient was a 83-year-old Japanese man. All tumors occurred in the infrascapular region. Macroscopically, the cut surface of all tumors showed an ill-defined and whitish mass with yellowish foci. Microscopically, the tumor consisted of collagen fiber bundles, abnormal elastic fibers and spindle cells suggestive of fibroblasts. Elastica-van-Gieson and Masson-Trichrome stain identified abnormal elastic fibers and abundant collagen fibers in elastofibroma, respectively. Immunohistochemically, fibroblasts were positive for CD34, but negative for alpha smooth muscle actin and h-caldesmon. Additionally, the cytoplasm of many fibroblasts was positive for TGF-beta 1 in all tumors. Ultrastructurally, some fibroblasts with abundant organelles in one tumor were observed in the adjacent area to amorphous elastic mass and bundles of collagen fibers. However, no myofibroblasts were also ultrastructurally identified in the tumor. Finally, our study supplies a further evidence that elastofibroma may show the proliferation of CD34-positive fibroblasts and contain no myofibroblasts, and fibroblasts activated by the autocrine mechanism of TGF-beta 1 may produce both abnormal elastic fibers and collagen fibers.

Results of these experiments confirm an earlier demonstration that dietary phenolphthalein causes thymic lymphoma in p53 + mice and show that 1 ; phenolphthalein causes qualitatively identical results in this transgenic model regardless of route of oral administration, 2 ; phenolphthalein shows evidence of micronucleus induction in p53 + mice for up to 26 weeks, 3 ; phenolphthalein induced transformations in the in vitro SHE assay, and 4 ; bisacodyl in p53 + mice induces neither drug-related neoplasm, nor micronuclei in polychromatic erythrocytes, and did not induce transformations in the in vitro SHE assay. | Medical profession must change things itself Editor--Smith's editorial about health policy is timely, but I wonder about relying on the government to ensure good quality care and improve health.1 As Smith points out, globalisation and economic forces limit the government's ability to improve health. The recent white and green papers are good examples of the government's limitations. The white paper has not only imposed the principle of fundholding on the unwilling non-fundholding doctors but has broadened the scope of fundholding to include general medical services and prescribing. The green paper, although making much of inequalities, says little about how to reduce them. The fact that New Labour has followed, and extended, the previous government's policies should surely tell us something about the challenges in the new world. What is needed is not reliance on the government alone but the will and ability to change things ourselves. As responsible professionals we can address certain questions. Why do we continue with outdated practices.

The Biomaterials Cell Organ Therapies Special Interest Group SIG ; is comprised of approximately 40 members from both industry and academia, including a significant number of student members. Towards this goal, the SIG is organizing a tutorial and a symposium at the 2005 annual meeting in Memphis. The tutorial, "Methods to Characterize Cells in Contact With Materials: Gene Expression and Activation of Cell Signaling Cascades, " is chaired by Donald Elbert, from Washington University in St. Louis. The response of cells to materials is dictated by the activation of cell signaling cascades. Characterization of the state of cell signaling cascades relies on a variety of techniques that monitor changes in gene expression, protein phosphorylation, or the presence of GTP on G-proteins. In this tutorial, the application of some of these techniques will be described in detail. For the study of gene expression, the use of quantitative PCR, real-time PCR, and gene arrays will be discussed. For the study of phosphorylation state or the presence of bound GTP, the use of pull-down assays, immunoprecipitations, gene knockdowns siRNA and antisense ; , and chemical inhibitors of specific signaling cascades will be described. Speakers will include Donald Elbert, Shannon Hughes and Shelly Sakiyama-Elbert. The symposium, "Stem Cells and Progenitor Cells in Regenerative Medicine, " is chaired by Shelly Sakiyama-Elbert, from Washington University in St. Louis, and co-chaired by Karen Burg, from Clemson University. This is a burgeoning area in the field of regenerative medicine, and the interaction of stem progenitor cells with biomaterials is crucial to the success of these approaches. Background Transesophageal echocardiography TEE ; has been used recently to detect atrial thrombi before cardioversion of atrial arrhythmias. It has been assumed that embolic events after cardioversion result from embolism of preexisting atrial thrombi that are accurately detected by TEE. This study examined the clinical and echocardiographic findings in patients with embolism after cardioversion of atrial fibrillation despite exclusion of atrial thrombi by TEE. Methods and Results Clinical and echocardiographic data in 17 patients with embolic events after TEE-guided electrical n 16 ; or pharmacological n 1 ; cardioversion were analyzed. All 17 patients had nonvalvular atrial fibrillation, including four patients with lone atrial fibrillation. TEE before cardioversion showed left atrial spontaneous echo contrast in five patients and did not show atrial thrombus in any patient. Cardioversion resulted in return to sinus rhythm without immediate complication in all patients. Thirteen patients had cerebral embolic events and four patients had peripheral embolism occurring 2 hours to 7 days after cardioversion.
