Sir, Although Sjogren's syndrome SS ; is occasionally complic ated with tubulointerstitial nephritis as one of the extraglandular manifestations, the association of crescentic glomerulonephritis is not common [1, 2]. We report here a female patient with SS associated with rapidly progressive glomerulonephritis and myeloperoxidase-specific antineutrophil cytoplasmic antibodies MPO-ANCA ; . Renal biopsy revealed crescentic glomerulonephritis with scant immune-deposit, in addition to tubulointerstitial nephritis. She developed progressive renal failure, but this was reversed by steroid therapy and plasma exchange. Case. A 67-year-old woman was admitted to our department on October 7, 1997 for nasal bleeding and progressive renal dysfunction. One year prior to admission, she noted the numbness and livedo of lower extremities. On March, 1997, she was found to have proteinuria and renal dysfunction BUN 24.9 mg dl, creatinine 1.7 mg dl ; . One month prior to admission, she had noted recurrent nasal bleeding. The patient was admitted for further examination. On admission, her temperature was 36.6C, pulse, 80 beats min; blood pressure; 160 90 mmHg. Examination of chest revealed fine crackles present at the lower back of chest. There was livedo on the limbs. Immunoelectrophoresis of serum protein demonstrated no monoclonal band and cryoglobulins were classified as type III. High concentrations of serum MPO-ANCA was noted 603 ELISA U ml ; serum proteinase 3 specific antineutrophil cytoplasmic autoantibodies PR-3-ANCA ; were not detected. Urinalysis showed proteinuria 0.43 g day ; and microhaematuria. Chest computed tomography CT ; scan showed irregular opacities, interstitial fibrosis, and mild pericardial effusion. Schirmer's test demonstrated 5 mm right eye ; and 5 mm left ; tearflow at 5 min. Minor salivary gland biopsy showed chronic inflammation with lymphocyte infiltration and acinal atrophy consistent with SS. A renal biopsy specimen contained 24 glomeruli, 11 of which were completely sclerosed, and four of which had fibrocellular crescents. The investigations described here on the effect of site-directed mutagenesis of mammalian P450s 2B4 and 2E1 make use of evidence from other laboratories that the corresponding mutation in bacterial P450s interferes with the activation of dioxygen to the oxenoid species by disrupting proton delivery to the active site. In a report on the crystal structure of the P450cam active site mutant T252A, Raag et al. 1991 ; discussed oxygen activation in connection with two possible pathways for proton delivery via an internal solvent channel between threonine-252 and glutamate-366 or via a hydrogen-bonding network extending to the heme environment from the surface of the cytochrome. As evidence for such a critical role for the conserved threonine, camphor hydroxylation by P450cam Martinis et al., 1989; Imai et al., 1989 ; and fatty acid hydroxylation by P450 BM-3 Yeom et al., 1995 ; were greatly diminished by the mutation of this amino acid residue to alanine. Furthermore, replacement of the conserved threonine of P450cam by unnatural amino acids provided suggestive evidence for a role of the oxygen atom of the threonine hydroxyl group in hydrogen bonding with the water that functions as the ultimate proton donor to the peroxo heme-iron complex Kimata et al., 1995 ; . Because the oxidative deformylation of xenobiotic aldehydes to yield olefins and formate presumably involves the iron-peroxo intermediate as the active oxidant, we proposed that disruption of the pathway leading to the iron-oxene species fig. 1 ; would result in the loss of oxenoid-dependent reactions, such as hydroxylation, and enhancement of deformylation reactions. As described below, such proved to be the case with P450 2B4 T302A Vaz et al., 1996 ; . Furthermore, an examination of epoxidation reactions with P450 2E1 T303A revealed a likely role for hydroperoxo-iron as an alternative electrophilic oxidant Vaz et al., 1998. Which is a fact so obvious that it seems to have escaped the attention of even the brightest and best of the controllers and policy makers whose job it is to contain all that chaos. But it hasn't escaped the Discordians. Real reality, say the Discordians, goes deeper that a subjective view of the world through a one small window of perception.
