Selection of patients for cardiac transplantation in the current era of HF therapy. Recent advances in therapy have greatly improved the clinical course of HF patients. To determine how these changes might affect the selection of patients for cardiac transplantation Butler et al. 26 ; evaluated the relationship between survival, peak exercise VO2, and Heart Failure Survival Score HFSS ; in the current era of HF therapy compared with survival post-cardiac transplantation in 320 patients followed from 1994 to 1997 past-era ; and 187 patients followed from 1999 to 2001 current-era ; . Outcomes were compared between these two groups and patients who underwent cardiac transplantation between 1993 and 2000. The investigators found that survival with medical therapy in the past era was 78% at one year and 67% at two years compared with 88% and 79%, respectively, in the current era both p 0.01 ; and that survival was improved in the current era regardless of peak VO2. Of interest was the finding that with current therapy 55% of patients with peak VO2 10 to 14 min kg had a low-risk HFSS and that this group exhibited an 88% one-year event-free survival which was similar to results reported with cardiac transplantation. These results suggest that improved survival for HF patients in the current era should lead to re-evaluation of the criteria used for listing for cardiac transplantation.
Hypothyroidism should in fact differ from the general laboratory reference range. A survey of U.S. endocrinologists and primary care providers published in 2001 found no consensus either within or between these physician groups as to what the target TSH range should be for T4 treatment 9 ; . Although there is some controversy as to whether the upper limit of the TSH reference is inappropriately high 10, 11 ; , an upper limit between 4 and 5 mU liter remains widely used. Only one previous study has examined the symptomatic effects of small adjustments of T4 dosage in patients with treated hypothyroidism 12 ; . In this study, participants reported improved well-being when T4 was given at doses that suppressed TSH concentrations. The study was open label and not randomized, and its results might be explained by a placebo effect. We therefore conducted a double-blind, randomized clinical trial with the aim of determining whether adjustment of T4 dosage aiming for a target serum TSH in the lower part of the reference range or below resulted in improved well-being and reduced symptoms of ill health compared with a target TSH in the upper part of the reference range.

I was born in 1940 in Vilkaviskis, and I graduated from Vilnius University in 1965. I've monkeyed about, and continue monkeying about, in various cultural spheres: I've been a teacher of Russian, an ethnographer, a philosopher; and I an art critic, a lecturer at the Vilnius Art Academy, a poet, and an essayist. As Giedra R. has remarked correctly, my life is utterly interwoven with texts ones I've both written and read and at times it's even difficult to tell them apart. For example, I have done some half-arsed drinking in my day. Since the vodka was of poor quality and my organism is a finicky one, I ended up doing my fair share of vomiting.
