Before measles immunizations were available, nearly everyone in the United States got measles - 3 to 4 million measles cases each year. An average of 450 measles-associated deaths were reported each year between 1953 and 1963. The Certificate of Immunization record card will be supplied by the Department of Health to each school district. The front side should be used to record immunization information. The reverse side of the Certificate of Immunization card will serve as the written exemption. If a parent is claiming an exemption, only that side of the card needs to be filled out. The exemption card should be filed so that the exemption statement is facing front. The wide line at the top of the exemption side of the card will make identification of the students claiming religious and or medical exemptions easy in the event of a disease outbreak requiring exclusion of students from schools and for tabulating the yearly reports. The minimum required immunizations are shaded on the Certificate revised in 2001. The Certificate of Immunization record card should be filed separately from the student's official record folder. A file box would be a suitable container for the record. The records will then be readily available for auditing or for checking for exemptions in the event of a disease outbreak. A Rhode Island judge has ruled that a reporter for The Providence R.I. ; Journal who took notes on a murder case is not required to give them to defense lawyers, but has asked the newspaper to give him copies of the notes for private review. A lawyer for Derick J. Hazard, a convicted murderer seeking a new trial, sought a subpoena for notes taken by Tracy Breton, the Journal reporter. But Superior Court Judge Edwin J. Gale dismissed the request as a "fishing expedition" and said the reporter did not have to turn over the bulk of her notes to the defense lawyers. The judge nevertheless said he wants to review the notes privately to determine their utility in a potential new trial for Hazard, who was convicted in 1996 of shooting and killing David S. The current understanding of the aetiology of melanoma considers ultraviolet radiation to be a major aetiological factor. What is not yet understood is the exact component of the UV wavelength responsible for melanoma, or the exact quantum of ultraviolet radiation required to bring about malignant transformation of the melanocyte. However it is quite possible that increased exposure to UV on Mediterranean holidays, particularly in early childhood, could be associated with an even steeper increase in incidence of melanoma over the next decade. Klapheke, MD bjective: On the immediate horizon is composite tissue allotransplantation CTA ; to restore missing limbs and to reconstruct the faces of persons with devastating disfigurements. The technical expertise exists, yet questions will remain in individual cases as to whether the potential benefits outweigh the risks, for example, of lifelong immunosuppression. Although there is extensive transplantation psychiatry literature, there are no publications on psychiatric aspects of CTA. The author details relevant psychosocial considerations to form the basis of psychiatric consultation in hand or arm transplantation. Methods: Technical issues in CTA, including information from the recent International Symposium on CTA, are reviewed. In an effort to find information relevant to psychiatric issues and evaluation of potential candidates for hand or arm CTA, a MEDLINE search was conducted utilizing terms of: hand, arm, trauma, amputation, transplantation, and psychology. Case examples seen by the author are reviewed for patients who have suffered upper-extremity trauma and for the first patients presenting for hand or arm pretransplantation evaluation including psychiatric consultation. Results: The author describes technical aspects and the process of proposed hand or arm transplantation, including indications, risks, benefits, and alternatives. Differences from other non-CTA ; forms of transplantation are presented, and issues involving the psychology of the hand and arm, issues of identity, and ethical considerations are reviewed. The specific role of the transplantation psychiatrist is defined and described in case examples. Conclusions: CTA is a new and different form of.
